Important Note

If the patient is a client of Wellness Way (will have DO NOT register form with them), register as Client Wellnessway and ordering provider of Jeremy Maxwell.  

If the patient does not have any other lab work and the patient or other company/provider is responsible for the collection fee, register the patient as Client Direct Access Testing.  If the patient is responsible for the collection fee, it is $20 (cash or check) per specimen type. 

If the patient has other lab work to be done, the patient will be registered as normal, and a MISCBLD order can be entered with comment of Kit collection to allow for patient labels for the specimens.
